According to the Rome IV criteria, in order to diagnose levator ani syndrome, patients have to meet all of the following criteria for the past three months, with symptom onset at least six months prior to diagnosis
•Chronic or recurrent rectal pain or aching
•Episodes last at least 30 minutes
•Exclusion of other causes of rectal pain
•Tenderness is elicited during posterior traction of the puborectalis
