• SV thrombosis without thrombosis of portal vein
  • Primary
    • Hypercoaguable/ thrombotic state
  • Secondary
    • Pancreatitis (chronic = most common cause),
    • Pseudocyst
    • Inflammation due to posterior gastric ulcer,
    • Cancer of pancreas or stomach,
    • Idiopathic retroperitoneal fibrosis
  • Clinical
    • May present as UGIB due to isolated gastric varices (bypass)
  • Ix – CT, EUS, MRI
  • Mx
    • Most asymptomatic & require no Rx – Rx thrombotic condition
    • Splenectomy for bleeding varices
