- Preparation
- Ensure patient is on a flat surface
- Maintain spinal precautions if indicated
- Remove any bulky clothing or objects over the pelvis
- Landmarks and placement
- Locate the greater trochanters—this is the correct level for binder application
- Binder should not be placed over the iliac crests or abdomen
- If unsure, err on the lower side—better to be slightly below than above
- Application
- Slide the binder under the patient using a log-roll or draw-sheet technique
- Centre it over the greater trochanters
- Pull both sides firmly to compress the pelvis and secure in place with Velcro or buckle
- Compression should be snug but not so tight as to impair distal circulation
- Post-application care
- Reassess haemodynamics and limb neurovascular status
- Check for any signs of skin pressure or compromised perfusion
- Confirm position with pelvic X-ray if not already done
- Leave binder in place until definitive pelvic stabilisation or exclusion of significant injury
