- Division of ligamentous attachments
- Falciform ligament
- Divided from the anterior abdominal wall to the liver edge
- Coronary ligaments
- Reflected peritoneum attaching the liver to the diaphragm; divided to mobilise right and left lobes
- Right and left triangular ligaments
- Formed by fusion of anterior and posterior coronary ligaments at the lateral liver edges; divided to free respective lobes
- Mobilisation of the right lobe
- Retract liver medially and divide the right coronary and triangular ligaments
- Blunt or sharp dissection to free posterior liver from diaphragm and retroperitoneum
- Take care near adrenal gland and IVC posteriorly
- Mobilisation of the left lobe
- Retract liver to the right, divide left coronary and triangular ligaments
- Elevate left lobe to expose oesophageal hiatus and left crus
- Short hepatic veins
- Found between posterior right lobe and IVC
- Need to take if fully mobilising the right liver
- Carefully divided between clips or ligated to avoid bleeding
- Caudate lobe mobilisation
- Involves division of small veins and fibrous tissue tethering it to IVC
- Important for full mobilisation during major liver resection
