- Needle insertion
- Insert a needle (e.g. 18G) just above the pubic symphysis under ultrasound guidance or by palpation
- Aspirate to confirm entry into the bladder (free flow of urine)
- Guidewire placement
- Pass a flexible J-tipped guidewire through the needle into the bladder
- Remove the needle, leaving the guidewire in place
- Catheter placement
- Place the trochar
- Remove the trochar and the guide wire leaving the sheeth in-situ
- Insert a Foley catheter through the dilator sheath
- Once in the bladder, remove the dilator sheeth
- Confirm urine flow through the catheter
- Inflate the balloon with sterile water (usually 10 mL)
- Securing and aftercare
- Secure catheter to abdominal wall with adhesive or suture
- Apply a sterile dressing
- Monitor for adequate drainage, catheter function, and signs of infection

- Slide the cathtre over the tochar
- Remove the trochar
