Pts who are heavily jaundiced from an obstructive cause usually suffer a specific form of coagulopathy due to a specific deficiency of Vitamin K, which is necessary for the synthesis of prothrombin (Factor II), Factor VII, Factor IX, and Factor X.

With biliary obstruction, bile salts are prevented from entering the bowel lumen, where they are required for the absorption of dietary vitamin K. Part of the role of bile is to help with the breakdown of fat into micelles that can be more readily absorbed by the GI tract (emulsification)
