Definition
- Inability to retract foreskin to expose glans
Aetiology
- Congenital due to physiological adhesions
- Acquired due to recurrent balanitis
Pathophysiology
Congenital - physiological adhesions between epithelial layers of inner foreskin (prepuce) and glans Acquired - due to scarring distally from recurrent balanitis
Clinical
- Paediatric - inability to retract foreskin for cleaning/bathing
- Adults - recurrent UTI, haematuria, painful erections
Investigations
- Not really helpful as this is a clinical dx
Management
- Congenital - At 5 yrs of age can trial 6 weeks of topical corticosteroid
- Acquired - Circumcision
Natural Hx
- By age 3 most foreskins become retractable (10% have physiologic phimosis at age 3)
